- Relocation Assistance (based on eligibility) -This position
will support the Infectious Disease HIV service. The nurse will
collaborate with a multidisciplinary team within Duke Health to
include Social Services, HIV Rapid Response Reengagement team
(H3RT), as well as Duke Well in order to coordinate care for HIV
patients that are out of care (OOC). -This role will also assist in
collecting data on barriers to HIV care access within Duke Health.
Following and completing phone calls to OOC patients ensuring that
they have follow up schedules with an Infectious Disease provider
as well as perform other related duties incidental to the work
described above. Duties and Responsibilities of this Level
- Plan and provide professional nursing care for patients in
accordance with physician orders and established policies and
procedures to include collecting and analyzing patient data,
developing and implementing nursing care plans, establishing
interpersonal relationships with patients, evaluating nursing care
given and communicating to proper personnel relevant facts about
patients, action taken and patient responses.
- Assist in the development and implementation of general
policies and procedures designed to effect efficiency of
operations.EducationWork requires graduation from an accredited BSN
program.Experience
- Twelve months of appropriate clinical experience is
required.
- Preferred 3-4 years of experience
- Preferred Infectious Disease and Care Coordination
experience.Degrees, Licensure, and/or Certification
- Must have current or compact RN licensure in the state of North
Carolina or Compact license from participating state.
- BLS required, but may be updated during orientation as need
be.Knowledge, Skills, and Abilities
- Ability to independently seek out resources and work
collaboratively
- Ability to establish and maintain effective working
relationships
- Ability to communicate clearly with patients, families,
visitors, healthcare team, physicians, administrators, leadership
and others
- Ability to teach patients and families in accordance with the
nursing plan of care
- Ability to record activities, document assessments, plan of
care, interventions, evaluation and re-evaluation of patient
status
- Ability to use computer and learn new software programs
- Able to document and communicate pertinent information using
computer and/or paper documentation tools
- Ability to navigate the entity to provide clinical care for
patients
- Ability to remain focused and organized
- Working knowledge of infection control procedures and safety
precautions
- Working knowledge and completion of appropriate The Joint
Commission (TJC) and other regulatory requirements - - Duke is an
